The perspective is looking upward toward the Bat symbol, which is an allusion to the title. That the buildings are crumbling suggests that Gotham City is in worse shape than we’ve seen it so far, desperate for a champion–even if that champion is currently on its “do not call” list. I dig this poster mostly for its simplicity.
Incidentally, Tom Pinchuk at Comicvine.com has posted that he believes this poster alludes to the “No Man’s Land” story from the comics. It’s certainly possible. Speculation that the film would draw on “Prey” from “Legends of the Dark Knight” 11-15 after it was announced that Catwoman was going to be featured drove up secondary market prices of those issues, so if you’ve got any “No Man’s Land” gaps to fill, you might want to either snag those missing issues now or content yourself to wait.
Haven’t really read any Batman comics so I don’t really feel one way or the other about what they’re adapting from. What’s got me excited is some of what the poster is suggesting and what you’ve said about Gotham being in worse shape without it’s champion. Seems quite ripe for thematic weight, and Nolan hasn’t steered us wrong yet.

If the story is adapting materials from “No Man’s Land” and “Knightfall” Batman is going to need a hell of a lot of help.
